N-terminal Pro-B-Type Natriuretic Peptide is a Myocardial Biomarker in Pulmonary Sepsis and Septic Shock

The objective: to study changes and prognostic significance of the blood NT-proBNP in patients with pulmonary sepsis. Subjects Methods. included 34 aged 54.5 ± 2.9 years sepsis or septic shock. Lethality intensive care unit (ICU) was 47.1%. NT-proBNP, procalcitonin (PCT) levels, lactate hemodynamic parameters were registered on 1st day (stage 1) 4th-5th ICU stay 2). Hemodynamics assessed through transpulmonary thermodilution. differences considered statistically significant at p < 0.05. Results: At stage 1, level 5,220 [1,380‒17,850] pg/ml, did not decrease ( = 0.726) 2 amounted 1,760 [631‒847] pg/ml. correlated extravascular lung water index (rho 0.445; 0.038) systolic artery pressure 0.414; 0.023). 2, PCT 0.569; 0.003), 0.525; 0.001), mean arterial norepinephrine dosage ratio -0.422; 0.035). no predictor lethality ICU: OR 1.0000; 95% CI 1.0000-1.0001. > 4,260 pg/ml (sensitivity 87.5%, specificity 94.4%) a lethality: 1.0004, 1.0000-1.0008, 0.046 (AUC 0.893, 0.732-0.974). Conclusion: Patients are characterized by NT-proBNP. biomarker hypertension moderate edema lethality. indices infection severity (procalcitonin, lactate, pressure/norepinephrine ratio). this stage, levels greater than 4,000 pg/mL and/or any degree both sensitive specific predictors lethal outcome. Specific features etiopathogenesis BNP hyperproduction make it difficult interpret elevation as an indicator cardiomyopathy but does reduce its value
